I have finally found a solution for this issue. It was in fact my sound card. Taking it out resolved the issue entirely. Thank you @dwd999 for being the first to mention that. Sorry for making you all wait so long.
It seems to work with my phone's bluetooth, but it's hard to tell as phones aren't exactly configured for this sort of thing. I'll see if I can track down a known good device and test it with that. I have a switch and I think it has a surround sound mode, so I'll see if I can get that working...

The speakers are set to 5.1 on both windows and the soundcard. I believe the setting is also correct on the AV Receiver. When using other input sources (such as bluetooth) all speakers operate.
The correct audio device is selected in the windows settings.
