I had the same problem. The following fixed it. Step 1: Go into your motherboard's bios and set VCCSA to 1.1V. Then download/install MSI Afterburner and enable Force Constant Voltage and Unlock voltage Control. Step 2: Use your card for a while and see if you still are getting black screen...

I suppose I could remove my water-cooler and then remount the retail fan/heatsink. I could then then RMA it and hope they don't reject my warranty (because of my custom water-cooler install). But even if they don't reject my warranty, its then a roll of the dice whether I get a better or worse...
