Update: after replacing the motherboard and gpu, it is still happening. I will try and rma the PSU and see what happens.

Update: Both my Motherboard and GPU are under warranty still so I am going to RMA them, and replace both, see if one of those was faulty. I will post an update after I get the new board and card.
