100% disk usage?!!?!?

So this just started happening I've literally tried everything I could find online to fix this and nothing is working. I just want to know if my hard drive is broken or what? I have a amd 4300 cpu 8gb ram and a 500gb hdd. I just reinstalled windows and that still didnt fix the problem so any help is appreciated. oh also Windows modular installer seems to be the thing using all the disk but its not just that theres like 4 or 5 things that keep bouncing up and down in the usage.

It's helped some people, but not everyone experiencing this problem. Other causes can include a failing hard drive (download and run manufacturer suggested disk utility software, SMART check etc), competing (ie Defender + other) or corrupted antivirus software, actual viruses infecting your machine, AHCI mode failure, and stuck Windows processes such as Windows update and WMP indexer.

I'd follow both of the suggested solutions above. Make sure your HDD is healthy running its brand-specific diagnostic utility or the third-party tool that @fry178 mentioned. Here's another thread that discusses the Windows services you should try disabling. I'd also advise you to check in the Resource Monitor/Task Manager what processes have been using the HDD and causing the spikes. It's also a good idea to run an anti-virus/malware software on your system and check if there are any concerning infections.

If the 100 % usage is still present after all the troubleshooting, I'd definitely try swapping the SATA cable and the SATA port connecting your 500 GB to the motherboard. A failed connectivity could also influence the HDD's usage & performance.

So I ran a full system scan and even a scan on the hard drive with AVG and it found no error or viruses so I know that's not it. there where some registry errors and things like that though. The problem is definitely better now its not at 100% the whole time but my memory usage it at like 43% non stop is that normal? I'm going to try and disable some of those services and see if that does anything. Whats weird to me is its saying Google chrome is the program eating up the memory its sitting at like 280 mb
 
Is this an upgrade from W7 or W8? Did you clean install? Have you recently installed the W10 anniversary update?

High RAM/CPU or Disk usage is a common 'bug' in the upgrade. There are methods to limit the impact, but I'm not aware of any that fully resolve the issue permanently.... other than clean OS install.
