100% Disk Usage

I'm not entirely sure what specs I need to supply you guys with but I've got 2 SSDs that have my OS and Select Steam games on respectively, I also have One 1Tb HHD that has all the rest of my junk on it.

This HHD as of Yesterday night has started acting up, It is currently sat at 0% usage because I have not accessed any files on it, once I try to download anything, or try to access a file or folder on the HHD, it immediately shoots to 100%. The usual noise the HHD makes when working "ticks" or "grinds" or whatever the noise it resembles, happens every other second, usually in time with the Steam download analytics graph.


Playing any game on my HDD sometimes causes the game to not run at all or causes micro stutters and occasional freezes. I tried to download War Thunder on a friends recommendation, I am currently unable to download this game successfully and usually quickly with my fast internet due to this HDD issue, It also severely bottlenecks my download to essentially nothing.

I'm not sure what's caused this but would love some help in solving this issue.
 


I'll do a test, but I'm sure the "tick" sound I'm referring to is the noise the HHD makes when it's being accessed and or working, it's just that it isn't a constant like when you're transferring files or downloading to the HHD, it's just sounding off at 1 second intervals like it's trying to work up the power to write and immediately falling short.
 


Well the HHD makes a noise when it's working, doesn't it? It's just that noise is a little more refined and slightly louder than what I previously remember...
