100% HDD even tho OS is in SSD

I have just built a new PC. However, my HDD goes up to 100 active time no matter what I have done (disable superfetch, index or scan fix etc) I thought the HDD is broken so I bought a new one but the problem still insist .. 100% again and again. I have test some diagnose but it said everything is fine with HDD. I really have no idea what the cause of problem.
Pls note that my OS is in SSD (C) and i plan to put game/data in HDD (D). Window may not be a problem because OS is in SSD and I have also tried disable lots of stuff but still 100%.
Pls need urgent help

*100% even I just right click on HDD, make new folder if start program it will take forever to load*

ok so after a long day of searching and testing I have finally able to get the answer and conclude this.
For anyone who also get the same problem and happen to stop upon this thread you may have a hope.

The problem of mine is none other the HDD itself - not OS -not Motherboard -not cable.
OS cant be a problem since it is in SSD.
Motherboard is out of question cause she works fine but cant make HDD connect with her smoothly so she put on the warning of disk error. if your motherboard have S.M.A.R.T dont forget to enable it -if it gives warning about HDD that is mean 90% that your HDD is broken.
Cable can also sometime give similar issue but only around 5% chance so if you get the same HDD error after your newly build check HDD first...

I'd strongly recommend you use a different SATA cable and check how your HDD would perform if you plug it to another SATA port on the motherboard. I suspect that the issue is most probably coming from a failed connection to the drives, rather than a failure in the drives themselves. Have you tried connecting it to another PC as well? Maybe you should give it a shot.
I'd also suggest you make sure you are running the latest BIOS version and you have installed the latest SATA controller & Chipset drivers from your motherboard manufacturer's official website.

I have updated everything to their latest. Well when you talk about cable it reminds me that I have one cable left after put everything. If i wrongly use SSD cable with HDD can it be result that that 100%? i have tried change port so far but nth work.

You can swap the cables, both the SATA SSD & the SATA HDD use the same connectivity. Use a different SATA cable or find the spare one to see if that will improve the HDD usage spikes. A faulty cable can most definitely lead to such abnormalities. You mentioned that you ran an HDD diagnostic tool on the drive, can you share the SMART results?
I'd also suggest you try the drive in another computer if you have access to one and see what its performance will be there.
Do you have any software that is running from the HDD? Posting some screenshots from the Resource monitor could also be really helpful.

New HDD (completely new) doesnt mean it is not broken. In my case here I have tested 5 HDDs all give 100% unbelievable but it is true. and all test/diagnose give positive.
for not wasted much time try to test it with other pc if it also show 100% that is mean as for new one/refund.

Indeed, @Tendosai!

Even though the hard drive passes the diagnostic utilities' tests, that doesn't mean that there might not be any internal hardware damage on the HDD itself. Sometimes, such hardware failures cannot be detected by the software tools. However, there are many cases when the HDD spikes 100 % disk usage in Task Manager but in fact, a program or a system file/service is causing it. Such HDD usage can occur when there is malware/virus on the system or if the connection to the drive is faulty (a cable or a port).

Either way, I'm glad you were able to find the source of the issue and got a solution for it! :)
I hope everything works flawlessly once you get the replacement HDD.
