100 percent disk usage win 8.1

Disk at 100, task manager shows nothing, all 0, but total is 100.
Active time is 100, but disk transfer
Rate is 0.
Happens for 2 mins. Happens once in 10 mins.
Cannot click anything when this occurs. No viruses.
This stopped coming for a month, but it's back now.
Edit: I've tried replacing SATA and power cables. No success.

It is a Desktop. I will check it and udpate the result soon.
BTW when i scanned with CrystalDiskInfo, it showed 24 current pending sectors[RAW Value] [ also, same number for irrecoverable sector count], so i think my HDD might be failing. It is Seagate , just over a year old...
