Question 12v Rail to CPU runs over 13 v

I have an Asus motherboard, I7 third generation, 32 gb of ram. The computer, when left off and unplugged for a day, will start for a few hours with the 12v rail between 11.75 and 12.985 volts. Then it begins to rise above 13 volts, and eventually shuts the computer off.

I'd like to know if there is a software monitoring program that can hibernate or shut down cleanly the computer when the 12v rises above a threshold of 13.25 volts. I need a clean shutdown to avoid damaging the hard drive. The motherboard is newer manufactured, but was first available in 2012. The Ram is DDR3. The power supply is a Corsair CX500M.

Is that a CX500M with the CX and M lettering in green or is that all black lettering on white font?

The green lettering is an old model and not really good. The black lettering on white font model is better but still pretty low quality.

I would replace the PSU. Voltages doing that isn't normal and I would be scared for my hardware.
