32GB MIcro card will not format or read on newer phone or new laptop.

I have a 32GB microsd card. My Galaxy S5 will not recognize it and when I put it in an adaptor my computer will show the drive however when I try to open it is says there is no disk and windows says it cannot format it. please help.
