Each separate molex string is one rail. You must use both rails to power the card.
You must use the pcie 6 pin adaptor that comes with the card, it splits into two molex connectors. Then plug one connector to one string, and the other to the second molex string.
If the fan goes noisy there is a rubber plug on the fan to oil it. You might need a molex to sata power adaptor too.
It has been confirmed that this cheap-o power supply does actually power a HD-4870 and a 125 watt athlon64-X2 6000+ under full gaming loads.
In another system one is powering a FX-4170 and Geforce 560ti.
This budget power supply would be better. But you are depending on the rebate coming in.
