3rd monitor out of range

So i was tying to connect my third monitor but once i connected it it was out of range. Is there a way to fix that ? i have AMD Radeon HD 5700 Series graphics card, intel i3-4330 3.50 GHz
The card supports has 2 DVI port one HDMI and one DP.
Any advice ? I have ITX motherboard

The hd 5000 series needs an ACTIVE displayport to whatever you want adapter to work with more than 2 screens.

It's as simple as use any of the ports for 2 monitors and then use the displayport with an active adapter for the thrid one.

It's as simple as use any of the ports for 2 monitors and then use the displayport with an active adapter for the thrid one.

And whats an active adapter ?
