4091 reallocated sectors!?

Look at the raw value that's red. 4091 reallocated sectors!?
Value: 1; Worst: 1; Threshold: 36; Raw Value: 4091. Should this drive be replaced immediately? Thanks. Also, on bootup, the motherboard throws the error "ACHI Port 0 Device Error". Thanks in advance.
 
Dump those ST, get a new WD red or black.

My ST2000, with a mere 400 ON days, recently gave me a big RED X as reported by a HD benchmark util, FAILS IN 5 DAYS! I immediately shut it down, ordered a new WD, was able to copy the stuff over. Attempt to reformat and resuscitate failed.
 


Is this a data disk separate from the OS disk? If it is the OS disk I suggest WD Black RE4 - it is a little expensive but mine have lasted several years with zero problems. For just data go with WD Black or Blue.

No, data and OS are on the same drive.
 


Then my number one choice is WD RE4 (expensive and great reliability) and second choice would be WD Black, third choice WD Blue - All three will work well!

And whatever you buy please do regular and frequent backups to an external disk.
