Hi, This is a 9600GT. It doesn't use much power. I use one in a Q6600 based system with a 350 watt PSU with no problem. If you have any reasonable 400 watt PSU this is not remotely a challenge for the PSU. Install the card and enjoy. It will be fine. There are many monitors that can pick voltage off your MB (e.g. Everest Ultimate edition, sissoftware sandra, etc), your BIOS may also have a monitor with ability to shut down if the voltage is out of range.
Your 400 watt PSU confuses me. You said "Output +12v 12A, +5v 25A, +3.3V 12A."
That (very roughly) says 12 X 12 = 144 watts on 12v, 5 X 25 = 125 watt on 5V, 3.3 X 12 = 40 watts on 3.3V. 144+125+40 is not close to 400 watts. I think you left out a +12V number. See if you have TWO +12V. If not then double check this is a 400 watt PSU. (I think this is what the first appender was pointing out)
This is the ratings on a good 420 PSU "+3.3V@20A, +5V@20A, +12V1@17A, +12V2@17A, -12V@0.8A, +5VSB@2.5A" Compare it to your 400 watt PSU, there are many amps missing on +12V.
