A GPU problem?

I have the GPU's overclocked with MSI afterburner with +150 for the core clock and +400 for the memory clock. I don't have any temperature problems and both cards work perfectly fine running Valley and Heaven Benchamarks and other stress tests that I've done for hours. All games, except Witcher 3, look and perform amazing too.

However if I try running 3D Mark, the benchmark always crashes on me. In addition to this problem I have this weird graphic issues while playing The Witcher 3. If I look into the landscape and in some cut scenes I have this weird squares/rectangles that appear in some parts of the screen. I also get kicked out of the game after playing for around 4 hours. Any thoughts of what could be causing this?

Lower your overclock, mabye save your games and reinstall The Witcher 3

Any sort of crash/artifact shows the overclock is high, decrease 10 mhz of core clock more and see if it is stable, i'd recommend keeping the voltage same and OC it at stock voltage to as far as it allows.

Ok I'll try all of those things but isn't it weird that it only happens in The Witcher 3 and they are running perfectly fine in other games?

It crashes in 3D mark simply indicated the OC is a bit high to handle and -10 core clock won't make any noticeable difference in performance.
