Solution
The question we have here is if your motherboard features a PCIe x16 slot. While the chipset itself does support PCIe x16, it is not a "given" that the manufacturer included that slot on the motherboard.
I'll suggest downloading and running SPECCY https://www.piriform.com/speccy/download (the free version works for this) which will tell you all of your hardware including motherboard model (very helpful to us).

If it turns out that you do have a PCIe x16 slot available on your motherboard, the Forsa GT630 should work

From what I can find, you'll likely be at the minimum playable frame rates and settings but all seem to be "playable" on a GT 630 with lowest detail settings and frame rates ~30fps (what gamers consider the minimum). Reports do vary though with some systems featuring a GT 630 are reported as providing acceptable game play. GameDebate reports the following frame rates at lowest settings for a system with those specs;
Assassins' Creed IV from 22fps up to 50fps (32fps average) - system does NOT meet minimum specs as suggested by Ubisoft http://assassinscreed.ubi.com/en-no/news/news_detail.aspx?c=tcm:75-119821&ct=tcm:6-231-32
GTA IV from 21fps up to 60fps (40fps average) - does meet official minimum specs as suggested by RockStar https://support.rockstargames.com/hc/en-us/articles/200144866-PC-System-Requirements-for-GTA-IV
Crysis 3 from 20fps up to 45fps (34fps average) - does meet official minimum specs as suggested by IGN http://www.ign.com/wikis/crysis-3/System_Requirements

Note that in the case of Assassin's Creed IV, your CPU is below minimum specs also

All frame rates reported are at low resolution (1024 × 768) and lowest detail settings

Assassin's Creed Black flag is reported to work by some even though the "minimum" specs are not met - I'm not so sure about the CPU situation though since I looked it up through graphics performance only.
This page from PlayTool explains the different slots http://www.playtool.com/pages/vidslots/slots.html used and what they look like (on your machine, slot 2, PCIe x16) is the one you'll want to use (regardless of graphics card chosen)

While I was considering GTA IV when suggesting the HD 6450 (brand really doesn't matter), it does seem to put you at the minimums for the Assassin's Creed versions you are asking about graphics wise, there is a question about the CPU in my mind though since the Pentium Dual Core E2220 ranks one tier below the minimum CPU suggestion of a Core 2 Dual core E4300 found here http://www.tomshardware.com/reviews/gaming-cpu-review-overclock,3106-5.html
I imagine it has to do with the included instruction sets of the CPU (yours has very few) since your CPU is "fast" enough in theory.
The HD 6450 is 5 tiers above the HD 5450 so it's definitely an improvement
