AMD FirePro W5000 Black Screen

Hi

I install AMD FirePro W5000 in my pc in PCI Express 3.0
when i start the PC every thing went fine I mean I see the motherboard logo then Win8.1 Starting Logo But after this I get Black Screen? and some times I see the main Disktop icons screen then i get blue vertical pattern lines or black screen? I did not get any driver CD with my Card box? now i remove the FirePro W5000 card to write this message I return to use built in intel HD 4000 my motherbord is (Gigabyte GA-Z77X-UD4H) running Win8.1 Pro 64bit I use DVI cable

after win8.1 pro 64bit finish starting I get this pattern lines or a just black screen

Memory: Corsair Vengeance 16GB CML16GX3M2A1600C9
SSD: Samsung 840 Pro 256 GB
HDD:7 years old Seagate 240 GB
Motherboard: Gigabyte GA-Z77X-UD4H
Graphics Card: FirePro W5000 Workstation Graphic Card
CPU Cooler: Cooler Master Hyper 212 EVO
Power Supply: PC Power and Cooling Silencer Mk III 600W Bronze

Where did you buy that card from? And is there a specific manufacturer for that or is it from AMD themselves? Try working with the seller, and try keeping to try to fill out the RMA form? Have you also tried calling them directly?
I use my PC for year and a half works Great.. but now I return using the built in intel HD 4000 after I Failed to make my FirePro W5000 Graphic Card to work

I understand that you have trouble installing the graphics driver correctly in system running Firepro W5000. If this is incorrect, please let me know as the information provided may change.
The problem could be a corrupted graphics install file in the OS that prevents newer driver from installing. Please use driver cleaning tool to completely remove all existing graphics files and update to our latest Catalyst 14.12 to fix the problem.

a. Removing drivers:
1. Right click on the left bottom of the desktop (Click the start menu) and Open the control panel then Programs and Features or Uninstall a program.
2. In the list select AMD Catalyst Install manager then click change.
3. In the window that opens click next and then select the option ‘Express Uninstall All AMD Software’.
4. Once this process completes restart the computer.

b. Use third party tool to remove the existing driver. Before beginning the uninstall/reinstallation process, please run Windows Update to ensure that there are no outstanding recommended/required updates, especially updates for the .Net or C++ frameworks, or Service Pack updates. This will ensure that our software will run properly. You may need to run Windows Update and restart multiple fDtimes, as certain updates will not appear in the list of updates until certain previous updates have been installed first.

Driver/registry cleaning tool-Driver Fusion:
1. Go to the following link and download and run driver fusion:
http://treexy.com/products/driver-fusion
2. Run this software and choose the option to select multiple cleaning filters, select all of the ATI filters and then click start. You have no need to precede the pay edition of cleaning and the free clan should be enough.
3. Once the cleaning has finished restart the computer.

Note: Driver Fusion is in no associate or maintained by AMD and is used at the user’s discretion.


c. Run the downloaded installer package choosing the option for express install. Once the installer completes restart the computer.

d. If the above doesn’t help, please update your BIOS and chipset driver for best support

AMD has the worse service after sale why nobody warn people about this thing
I lost $350 in this Faulty Card
