what's happening? when does it run? you can use AutoRuns to look at what gets launched at login and stuff. scan for virus and malware first

It just pops up out of the blue 2-3 times a day. I just ran autoruns and didn't see anything with that icon. I will wait till it pops up again and then run.

Sure, the file might be something else, but I would run the below and check windows out

right click start button
choose powershell (admin)
type SFC /scannow and press enter
once its completed, copy/paste this command into same window:
Repair-WindowsImage -Online -RestoreHealth and press enter
SFC fixes system files, second command cleans image files, re run SFC if it failed to fix all files and restart PC

The other file mentioned by Gardenman sets up a profile for the connection manager monitor
