Artifacts and Freezing/Crashing

My desktop has been artifacting and freezing mostly while playing videogames.
I decided to RMA my videocard (GTX680) after confirming it was faulty.
I received my new GTX680 today only to find out I am still getting artifacts and crashing.
They are a bit different however (terms of shapes and colours).
The temps and voltages are fine.
My PSU is Corsair RM650W.
I am clueless. Any ideas?

Memtest86+ passed.
 
Could be an overheating issue, or maybe your PSU failing (always a possibility even on a quality unit). Monitor your CPU/GPU temps and voltages with AIDA64 or HWMonitor Pro with the log feature on, so you can review the results after it crashes.

EDIT: how exactly are those temps and voltages? can you please post them?
 
So after last freeze my neighbor that was helping me decided to remove and insert the HDMI cable. I was completely surprised to see the freeze was gone and the computer was fully responsive.

Both the cable and the monitor work with my laptop, so, I think the culprit might be that the cable was being pushed by the wall resulting in poor contact. I haven't confirmed this, but as of yet, no crashing.
