ASRock Z77 Extreme4 USB stops working

HELP! My Windows 7 gaming computer's USB (2.0 and 3.0) stops working aprox. 5 minutes after I turn it on. Sometimes my UEFI doesn't see my SSD when booting up.
My setup is ASRock Z77 Extreme4, Intel Core i7-3770K, Crucial Ballistix Tactical 16GB (2 x 8GB), Mushkin Enhanced Chronos MKNSSDCR240GB 2.5" 240GB SATA III, Fractal Design Arc Midi R2 Tower with a Rosewill PSU (HIVE Series HIVE-750 750W), Asus GTX 660 Ti 1006 MHz Core - 3 GB GDDR5 SDRAM (GTX660 TI-DC2OC-3GD5).
This all started about a week ago. I built this system in 2013. Upon completion of the build, I noticed the USB 3 ports were as slow as USB 2, I didn't pursue this problem because the USB 3 speed wasn't a concern at the time.
Everything worked fine until aprox. a week ago.
In Device Manager I have error in Other Devices - SM Bus Controller driver unavailable.
Researching this problem I've found alot of folks with USB 3 problems, but not both USB 2 and USB 3 failing, and not after 5 minutes of operation.
Any ideas?

Solution
This MOBO has double controllers for the USB try plug the Mouse and Keyb to differents ones, the intel provideds by the z77 and asmedia at the same time, could be resulting in:
Only 1 chipset fails (RMA if possible)
They both fails (You will need to check another PSU)
Note: is the SSD connected to the Asmedia provider chip or the Z77 provider?

And download every thing that is on there, and you should unrar all files and burn them on a DVD or add them on a thumb drive in case you need it in the future.

Anyway, right now you only need USB driver, which is this one:

Unrar it, go to device manager>right click>update driver software and click browse and go to where you unrared it. I have a 990fx extreme3 and had the same problem, downloading the drivers helped me. About the SSD, i don't know what could be the problem, maybe this will fix it, give it a try.
 
You saw my other thread with same problem almost. Was a dead port on my PSU and I just plugged into another one. You re-checked all cables right? Like unplug and re-seat each one?
 
Thanks for the recommendations George and SweesyM. I have reseated all connections and reloaded all of the ASRock drivers but no change.
Wanting to isolate the problem as either a hardware or software problem, so I ran Ubuntu 14 in demo mode with none of the problems showing up. I ran Ubuntu for several hours without a hitch. Went back to Windows 7 and once again the USB kybd and mouse stopped working after 5 minutes. Any ideas?
Also, I'm new to this forum, so I hope I'm using correct protocol.

Do you still have the drivers disk that came with your MoBo? I remember installing every damn driver right after my windows install, and believe me it was a pain cause of all the restarts. There was option to install drivers individual or there was a package deal that kinda just ran and prompted you to restart when it needed human interface. My system still is not perfect by any means but when I had the USB issue it was PSU related. The strangest part was that the cluster of 5 case fans that I had running off one plug were dead...plugged it into another port on my PSU and bam! No more problems.
 
I don't have the disk that came with the mobo, but I went to ASRock's site (same place George suggested) and downloaded and installed everything they had - no change. I've also tried newer drivers for ASMedia. I also swapped to the other plugs on my PSU...no change. It's gotta be one of the drivers, or some other type of software, because it ran Ubuntu 14.04.1 in demo mode with no problem, if their was a hardware problem, it should've affected Ubuntu the same way.
It just trips me out that Ubuntu works better than Windows, and it's free. I'd be running Linux 100% of the time except for the fact that all my games are Windows based.
What else has me baffled is that this rig was running fine for over a year, then all of a sudden, it starts acting up. Maybe it's a virus, but I'm pretty vigilant about keeping viruses off my rig.
I've got a couple of friends that are A+ certified, I'll be picking their brains for a solution.
I do appreciate your feedback.
 
OK, troubleshoot update...
While researching for answers with my rig in UBUNTU demo mode - it locked up after an hour. I launched Windows in safe mode - locked up after a couple of minutes. I disabled the USB selective suspend setting, I also turned off power management in all USB hubs in Device Manager - USB still hangs up after a couple of minutes. I have to retract my earlier statement, it probably IS a hardware problem. Isolating the problem to the defective equipment is going to be tough, all my input devices are USB, no PS2 devices. I don't have a spare PSU either. Any suggestions on how to isolate the PSU or mobo?
Any suggestions will be greatly appreciated.
 
Without a spare PSU your options are limited. You could manually test your PSU for the proper voltages but it is very hard to do.

Also don't forget to visually look at each plug to make sure none of the pins have been pushed back. Even check some of the 4 pin power connectors in and around your case. Many times we plug and unplug while checking our systems so many times and the plugs are of poor quality. You can help push any pins back forward with a small set of needle nose pliers with the power un-plugged.

Solution
I am looking almost the same problem that you are having except mine does not quit working all the way. It just makes the mouse work erratic and the keyboard it fine most of the time. So it's not just your mouse, it's the motherboard I am sure, because I have tried my stuff on other computers and it works fine.
