Asus Radeon R7 250X problem

Hey guys i have a problem with this card lately.
Monitor freezes doenst matter if i play or not but sound still playing.looks like pc still working but this appears everytime and i cant do anything besides turn off my pc.i have latest drvers and the connection to my monitor is via DVI.monitor looks like this:

I'll go ahead and recommend you a tempature monitor for your components . HwMonitor is a good light program to help you check your temps of all your components . Tell me your temps when finished .
Secondly run unigine valley to stress test your gpu and see if the screen goes weird . When does this occur for you ?

max temp on furmark 78oc no errors occured.temps seems fine.this is not happening while im gaming happens only in google chrome trying to see streaming or some random online video i think!
