If you've ruled out driver issues (fully removing with DDU, then reinstalling), then it's likely a hardware defect.

Is the card new? If so, I'd look to RMA/return the card.
That is a fairly common issue on defective cards, IIRC, it's due to VRAM issues/overheating.
