ASUS STRIX GTX970 only sends a signal to the monitor for a few seconds

My monitor lost signal from my graphics card earlier today. I ended up trying a few other connection types to see if it was an issue with my HDMI cable, but nothing fixed it.

I hooked the monitor up to another PC, and it worked fine. So, that told me that it wasn't a monitor issue.

I hooked the card up to another monitor and still no signal.

I pulled the card. Reset the BIOS by pulling the battery. Started up the PC without the card using the on board GPU and that works fine.

I put the graphics card back in, switched the cables to use the GTX970, it was working for about 10 seconds and then no signal. I powered off the PC and powered it back on, and this time the signal lasted for a few seconds.

The card has been working for a about 4 years with no issues. Any additional steps I should take to troubleshoot this? Unfortunately, my other PC is too old and doesn't have a PCIE2 slot to test the card.

Additional info about the PC:
MB: ASRock H97 Anniversary
CPU: Intel i5-4460 @3.2Ghz
RAM: 8 GB RAM

Any help is greatly appreciated.

System is about 4 years old. Some more details:

PSU: EVGA - SuperNOVA GS 550 W 80+ Gold Certified Fully-Modular ATX Power Supply
GPU: Asus - GeForce GTX 970 4 GB STRIX Video Card
CPU: Intel - Core i5-4460 3.2 GHz Quad-Core Processor
RAM: G.Skill - Ripjaws X Series 8 GB (2 x 4 GB) DDR3-1600 Memory
HDD: Samsung - 850 EVO-Series 500 GB 2.5" Solid State Drive
MB: ASRock - H97 Anniversary ATX LGA1150 Motherboard

I'll see if a friend or family member has a PC where I can try the card.

Tried it in another PC with the same result. Signal is sent long enough to see the BIOS screen then to see the Windows loading screen.

Other PC is quite a bit older:
MB: Asus - M4A79XTD EVO ATX AM3
CPU: AMD - Athlon II X4 620 2.6 GHz Quad-Core
RAM: G.Skill - ECO Series 4 GB (2 x 2 GB) DDR3-1333
PSU: Antec - EarthWatts Green 650 W 80+ Bronze Certified ATX Power Supply
GPU: Sapphire - Radeon HD 5770 1 GB (this is what I swapped out)
