Audio out of headphone and speakers?

I have been hearing sound from both my headphones and my speakers. Sometimes I also have trouble plugging in my headphones(3.5mm, not the usb) and getting them to work. Can you help me out?

Have you tried right clicking the audio icon on your taskbar, and selecting "Playback Devices", then selecting the headphones or speakers and setting them as the default device?
