Bad Graphics Card?

When I am gaming, but only occasionally, My monitor will go blank, and my secondary monitor will go blue with stripes of different blue going down it. I was wondering if this could be the card overheating/failing, or my hard drive, which I know is failing, I'm trying to squeeze a few more days out of it (not storing any important info on it, have reinstalled windows 3 times). It didnt do it before the new card ever, but before that the hard drive wasnt that bad. I have also just replaced the PSU, could that be a factor? Only reason i say hard drive is because when it freezes, my hard drive activity light gets stuck on, and it is seemingly random (while playing league of legends)

Install furmark on your pc, let it run for at least half an hour. if the gpu temps went up it will show and the crash. if not, just reinstall windows on a different hard drive.

I've ran furmark, and it heats up, but no crash.
