i believe its MSI Afterburners On Screen Display. its a function of MSI Afterburner, a AMD/Nvidia Overclocking tool

Start MSI Afterburner up

Click Monitoring
Choose which you want to show in active monitoring graphs (click on them then tick the On Screen display).

It is definitely MSI afterburner. You can also link it to HWinfo to get other stats like CPU temperature and fan speed.

I use it to keep an eye on my hardware while running games.
