I would like a graphics card that can run on a 250w power supply and that can run games like minecraft with maxed settings and shaders and MAYBE games like Call Of Duty Black Ops II on low to medium settings. And i need to make sure that it will fit in my PC case since it is a slim model of the original Dell Inspiron 560.

Solution
The only card that might possibly work is the GT 730 (25W) (the GDDR5 version). And that is without any guarantees. A 250w power supply that probably isn't the highest quality is really too small to support an external graphics card. 300w is usually the minimum.

I do sincerely apologize for bashing you. My intention wasn't to put you down, but I am not at all comfortable with that recommendation.
I did the numbers for a 750ti also and came up with 200W. Now that would be fine if his power supply were a Seasonic, but the power supplies that Dell puts in their cheaper lines of computers are mediocre at best and often can't push the wattage that they are rated at. I can't say for certain that it wouldn't work, but I also can't say with any certainty that it would.
But like you say the 750ti would be overkill for his system anyway.

I did a bit of digging and found what I believe is the power supply in your computer (or something close http://www.amazon.com/Dell-L250NS-00-Inspiron-Optiplex-Compatible/dp/B00CIXLUQA). The 12V line is rated at 18A which while below the recommendation should still work since your processor isn't power hungry.
I would go ahead and get the GT 730 and use it in your computer as is.

Actually i believe this is mine, my power supply model is a HIPRO D2503R001 http://www.amazon.com/Genuine-Dell-Inspiron-Systems-Identical/dp/B00CLV78NI/ref=sr_1_1?s=electronics&ie=UTF8&qid=1427388260&sr=1-1&keywords=D2503R001 If this is not it, its at least close, on the +12V line it says 17A and on the -12V it says 0.3A

That one Amp isn't going to make a difference. I think you're ok.
