Question black and green squares appearing on videos.lines appearing on screen.games looking incorrect

a couple of days ago i started having a problem first appearing on YouTube where black and green squares start appearing randomly on videos

and something like this too

and after a while these lines started appearing on my browsers and everywhere to be honest

and then my PC started crashing saying video tdr failure atikmpag.dll and it started crashing consistently i thought it was a hardware issue maybe the cable or something so i changed it but the problem was still there but the trigger seems to be every time i open a video. and also when i play a game these weird squares started appearing as well

so i don`t know what if this my card going bad or something so i really hope you can help

Which GPU is it ? List full PC specs. It looks like your Video card might be faulty, VRAM failure issue. Though, I'm just making an assumption now.

oh yeah forgot to list them its a pretty shitty rig that ive had for a long time
cpu:Intel Core 2 Duo E8400 3.00Ghz
gpu: Amd radeon hd 3600
ram:4 GB
