black screen when playing games

this has been going on for almost two weeks , my pc works fine during normal usage , internet browsing etc . but when i start to play any game , after sometime monitor display vanishes(black screen) and then comes back , this thing goes on randomly , any suggestions pls!!!!

xfx radeon hd 6950
8gb ram , 700w psu

might happen with all games , but for the moment i've experienced this on call of juarez gunslinger and fallout 4
