This problem has been occurring constantly over the past couple of months, some times it will disappear for a while and come back randomly.
What will happen is my monitor will turn black and after a couple seconds it will say "No HDMI connection" then reboot giving me "Windows has recovered from an unexpected shutdown".
When the problem occurs it will always happen after 10 to 15 minutes of playing a game like Battlefield 3.
I have been updating my drivers and some times I rolled them back to see if that fix the error. I think it might be my GTX 680.

Please I need some help.

I have read that it is video related "The code (BCC 116) simply means the video system could not recover. ....116's are usually heat. If it is a laptop it is almost certainly heat.
