[SOLVED] Boot and crash issue

In the last 2 weeks I have an ongoing issue when booting up my PC, it enter bios.
After few attempts to restart the PC it boot normally and I can do everything play, work etc... but my PC freeze like twice in a day and I have to restart the PC. it doesn't feel like its happening when the I overload the PC cause its happened to my also when I only use firefox(about 6 tabs).
I tried to monitor with the task manager and I saw when my PC freeze my disk was stock on 100% and also when the PC goes to BIOS it sometime recognize the SSD and sometimes not.
is that mean the SSD is 100% faulty or can be the SSD cable? maybe PS?

I also tested with HD Sentinel,

It's easy enough to reseat the sata cable both ends and the power cable.
See if it makes a diff.
