Booting issues with GPU

Ok so I was playing game (don't recall what game) and my system froze nothing would work and had to restart my computer. It was booting up and said windows had an issue and had to restart from a last save point or something (don't recall was over a week ago). It shut down and started back up. This time the screen was not fitted like it should be. I went into the options and reset the screen size (still didn't fit, but was better). The screen was having some tearing with pink lines on the screen. I looked at the options and it said my computer was trying to run at like 65 refresh and my monitor is a 120, but couldn't change the option. Tried to check drivers to see if it uninstalled it or something, but when I was trying to my computer would just not work very well. Was very slow programs kept going not responding. I finally was able to download and update drivers and it tried to restart my computer to finish the install. Now it wasn't booting up at all. Kept getting stuck at the black screen after the screen that says "press F12 for this press del for this"

Now I tried a few times to get it to boot, nada.

So I tried to pull out the GPU and it boots up fine like nothing is wrong. Computer is still acting a little funny sometimes not opening programs like it should. For the most part my computer is working without the GPU. I tried to download and install the drivers without the GPU inside and once it starts to install it says I can't because of my hardware (the GPU isn't inside so won't install). I tried a few mins ago to boot it up with the GPU and I got this lovely screen after the screen that says "press F12 for this press del for this" http://imgur.com/Uw1B0p0
I can hit enter and the _ will drop down a line at that screen.

I normally try to look around pretty hard or figure stuff out on my own... This time I've got no idea and could use a bit of help.

I wish it was EVGA but they were always sold out... so I went with ASUS. Filling out their claims right now
 
ok so I put in the ticket and everything. they replied telling me to use the right S/N, but I don't have the box anymore. The number I thought should be it I'm guessing isn't. to make it easier for help here is a picture of my GPU tell me the right number to give them. The picture isn't the best, so here is what they say by color.
