BSOD: Kernal Security Check Failure even after factory reset

I've been getting this BSOD for a couple weeks now.

I would get this bluescreen while playing Minecraft, using this program called FL Studio and just simply watching a movie. I've already done a complete factory reset, updated my bios, made sure all my drivers are updated, and even tested my memory using memtest86.

I think this is hardware issue, but I'm not sure what to replace. Please let me know if you have any solutions.

uninstall the usb audio driver, you should change the memory dump type to kernel so that info on the USB subsystem is saved in the memory dump file.
the memory dump file will be larger and located at c:\windows\memory.dmp
bugcheck was in the usb extension.

what is this file for? (I would remove it)

(looks like a usb headphone amplifier driver
maybe look for a updated driver from http://www.fiio.net/en/supports/18
(most likely cause of the bugcheck)

you also have some driver being installed from the wrong location.

OK I just did that. I will let you know if I continue to get bsod. Thanks
