BSOD Memory Management Win7 x64

my pc is 3 years old with these specs:

-AMD Phenom II X4-955 BE
-Biostar TA890GXE
-V-Gen DDR3-1333mhz Memory (2x2GB)
-HIS Radeon HD6870 1GB
-Seasonic 500w bronze
-Seagate 500GB SATA3

i got random BSOD recently, although i've reinstall Windows, after 3-4 weeks it came BSOD again. recently it has BSOD caused by Memory_Management 0x0000001a ntoskrnl.exe anyone has the solution? thanks in advance
