Can someone explain these S.M.A.R.T results?

So today I started getting random hangs and freezes on Windows 10 (with an especially large click from the PC on the first one), and I have narrowed the issue down to what I think is my D drive failing (not the one Windows is installed on). I came to this conclusion as whenever something tries to save to it (nvidia shadowplay) it begins to freeze and me doing a scan for errors for on it caused about 10 minutes of hanging. I can open the D drive in file explorer but after a little while it begins to hang and freeze windows up.

Does this sound/look like a drive failing and if so, how can I get the data off it without it hanging and freezing?
 
Thanks but I've had someone else tell me that's it's very bad (I'm getting conflicting opinions on this) and I'd like to know what the results actually mean? And if the hard drive is fine, would there be anything else causing the symptoms I've explained above? Thanks.
