Cannot format usb flash drive. [Help]

I hope someone can help me,
I have a Toshiba flash drive 16GB, when I made a bootable USB to install windows 7 on my laptop (I use Windows 7 USB DVD Download Tool), it failed halfway (my brother unplugs it, maybe, because I was not there) when copying the ISO file to USB, after that I close the program and reinsert my flash drive, and now here's the problem:

-my USB drive now NTFS,
-in windows explorer when I right clicked the flash drive directory (D: ) the windows explorer always became NOT RESPONDING, thus I cannot format it.
-and everything I did that has to do with drive D: always became NOT RESPONDING, like opening Disk Management, opening drive D: etc.
-I also tried to format with different ways like cmd format d: /f's: FAT32 but got stuck at 0 percent
-also guiformat or tried to search what program is using it, with Unlocker, process explorer, in safe mode, or randomly stopping service in task manager
-reinstall the driver
-googling.....

but none working, please someone help me.

Solution
If you can not access files on it, then it is damaged. So there is not much you can do . Lucky those things only cost about $8 to $10 at walmart :)
Thank you for your answer, I also have guessed it to be so, but I asked anyway, because when I tried to format it, it said the drive was being used by another program, and another said something about I/O.
 


No, I haven't, right now I'm trying to format it through disk management (65%), after I followed gilbadon's suggestion to use MiniTool Partition Wizard, I can access disk management again (before, it was always not responding).

Will try to format it through Linux OS as the last resort.

I'm sorry for the late post, the result was that the format failed, anything I did to it would fail so after some time the flash drive dead, no media.

But it's still fun to try to fix the problem, and I also get pretty useful info from gilbadon.

Thank you everyone, sorry for the trouble and annoyance, maybe, (because there are many similar threads with this kind of problem). And I need to buy 3 flash drive in case of emergency like this.
