Canon ip4600 cd case printing

I need help with my Canon ip4600 useing the CD lable print, the template for the cd lable is fine but I cant seem to find a template for the case inserts. That is if there is one?
when you go to the "Help" it does make mention of printing a jewel case (label out line, fold line, cut out lines, all that sort of stuff but no mention of template.
So is there a CD case isert template? Help me plese
