Can't find fifth ending in Slouching [SPOILERS]

Alright, I've finished playing Slouching Toward Bedlam, more or less
enjoyed it, but I remain unable to find one of the five possible
endings. I'm pretty sure it's Appendix A that I haven't seen, but I
haven't double-checked the lettering.

Endings I've gotten:

* Dying while infected people remain alive. (Appendix B)
* Preaching over the magnetophone. (C)
* Killing everyone infected and being committed to Bedlam. (D)
* Killing everyone infected and hanging myself. (E)

Things I've unsucessfully tried for Appendix A, all with various
permutations of leaving people infected, uninfected, or dead:

* Confiding in James (telling him about Cleve, madness, etcetera,
giving him Cleve's case file).
* Confiding in Du Monde (giving her various artifacts).
* Killing James during the first few turns then locking myself in
Corridor 6.


Things I've thought of but don't think are possible:

* Leaving some sort of record of my knowledge via Triage or the
phonograph.
* Finding some use for Du Monde's pin.
* Dying without ever infecting anyone.

So what am I missing?

Is it possible to kill the driver and get away to kill someone else?
Every time I type "kill driver", I am taken to an alley from which I
cannot escape.

You should be able to enter the cab after that and then drive yourself
wherever you like. If I recall correctly.

I don't think I tried that. It's a reasonable solution though. I think
what I did was just kill the driver last. Do everything else I wanted to,
having the driver take me to various places, and when I was in the last
place I wanted to be, kill the driver (if I wanted/needed to). You can
try doing it that way, too.
