Can't Format 64gb Class 10

So I wanted to format my class 10 64gb sandisk microsd card to Fat32 because I'm using Cyanogenmod ROM and it doesn't support ExFAT.

What I've tried so far:
-the switch on the Sandisk Adapter
-Using the format option on both OS X and Windows 7 for ExFat, FAT, Journaled etc
(on OSX it says couldnt unmount Disk for Disk Utility and so I rebooted and held command+s and it said it couldn't format it).
-Using SDFormatter for both OS X and Windows 7.
(it says format complete but the card and the files on it have not deleted.)
-Using Windows 7, deleted all the files on the card and it says deleted but when I remove it and reinsert it, all the files are still there.
-Using Disk Utility in OS X 10.10, I clicked "verify disk" and it found errors saying "Found directory entry beyond end of directory in:

So I click Repair disk and it says:

Back up as many of your files as possible, reformat the disk, and restore your backed-up files.

So then I click erase on Disk Utility with the option ExFat enabled and i get this error:
Volume Erase failed with the error:
POSIX reports: The operation couldn’t be completed. Input/output error


I've been doing research for the past week but can't figure out how to go about this. Any help is appreciated!

Edit: I've also used Fatformat32 on Windows 7 and that didn't work either.
 


No solution was found, you're under warranty so just send it back for a replacement that's what I ended up doing
