can't format sandisk extreme speed otg usb 128gb

I can't format my otg usb. 'windows was unable to complete format'. Also can't use in disk managment 'the format did not complete succesfully'. File system is RAW. ExFAT. Help me pls

Maybe a failed drive, test it in another system. See if a Linux Live disk will complete a format.
