can't format SD card

I have Samsung micro SD 8GB 6th class. It run on android phone like charm. But it gives me some problem. I'm unable to format it on my Lumia 520(8.1 cyan update) so I tried it on PC but wont work. Plz help me. Even PC can't format it.

First, scan it using Kaspersky ( for best ) or Avast or Avira... If there is no virus, check if the squarish supporter of the SD card's lock button is up or down, change its direction and repeat formatting.

It would be also great to see what it shows up.

There is a tab on all the cards makes sure its switch in the right position so you can format it.
