Cant get game to run at 1080p on a 4k monitor!

I recently got a 4k monitor (Asus PB287Q) and am having issues running Splinter Cell: Blacklist at its native 1920x1080 (I am using a GTX970).

The game loads as default to 4k, windowed, and is locked to 29hz (unplayable).

I would like to run the game at fullscreen ,at 1080p, at 60hz, But have not been able to change any of the settings to make this happen. The refresh rate is grayed-out and not able to be changed, the game always goes back to windowed if I change the resolution, and it is just not playable.
Any Suggestions?

Little bit annoying, but have you tried setting your windows desktop to 1080p and then loading the game? It may at least trick the game into letting you configure it properly.

I have tried that, but when I load up the game, and set it to full screen (it defaults to windowed) somehow everything goes back to 4k

Very Wierd!
