Solution
Start with DDU:
http://www.guru3d.com/files-details/display-driver-uninstaller-download.html

Just run that (will reboot) and it will end up removing all your video drivers. Next, just get the latest NVidia drivers.

If you still have issues, then there may be some data corruption in Windows somewhere.

Other:
I had major problems with missing content in Windows (e-mail, right-click in programs, no highlight of items in Start Menu etc) but I fixed it with DDU + NVidia reinstall.

It ran fine for a few hours, then I check MS Updates and saw it starting to install an NVidia Update (huh?). It finished and my problem came back.

I reinstalled my video drivers again, but thus far MS Updates hasn't screwed up again.
