So the issue ended up being what kari suggested. The video card would keep being used for things like streaming video and the usage % would keep going up and down. It started getting a little warm at idle again. Does anyone know how to make my system automatically switch to use the integrated graphics when in 2d and video card for gaming? After burner has a setting tab which I can select the intel hd integrated which is what I did and now the video card idles normal. It's a bit annoying to keep switching back and forth, there has to be a way to automate this.

Solution
maybe the card doesnt properly drop down to the idle clocks after gaming, see what GPU-Z tells you (sensors tab)

Just installed it and alt-tab between divinity and desktop a few times and the clocks went up and then back down 300 and 150mhz. I did notice there were random low spikes in card usage %. I run afterburner keeping a 20 minute history. It would randomly hit 20% or so out of nowhere then go back to nothing, but shouldn't the temps fluctuate up and down as long as its not constant usage? I did have several tabs open on youtube if that has anything to do with that. I also turned the fan up higher and it still stayed at the same temp. If fan went below 25% it would slowly go up another 1 or 2. It seems fine now, will see how it is when I completely shut off the game and go idle later.
