Hi,
I'm looking for the cheapest GPU that can handle at least two monitors:
  • 3840x2160@60Hz (DP, 2xHDMI)
  • 1920x1080@60Hz with sound (HDMI)
Currently, I'm using Intel i5 6600k with integrated GPU. Can you suggest any solution?

I use Samsung U32J590 and old Samsung TV. I know that max resolution for Intel HD 530 is 4096x2304@60Hz, but unfortunately it can be achieved only using Display Port, which my motherboard (ASRock Z170 Gaming K4) doesn't have. Otherwise max refresh rate is 30Hz for 4K resolution.
It's difficult to determine the exact budget. I don't want to spend more than 300PLN, which is about 75USD, but the fewer is better.

$75USD won’t be enough to buy a GPU that runs games at 4K resolution @60Hz!
In the current state of the GPUs market, you’ll need a budget almost 10 times bigger than that.

But I don't play any games. I am a programmer and my only requirement is smooth image display. Most of time I use IDE and browser, sometimes I work with 3D graphics (OpenGL), and HD 530 would be enough if only it can handle 4K@60Hz.
