computer freezing to solid colour

my computer has been freezing to a solid colour on one monitor for over a year now. it used to happen maybe a few times a month, now it's happening a minimum of three times a day. i have to restart my computer every time. i can't run anything, really (not even modded minecraft lol) without it happening. i tried to update my driver, but the first two times my computer froze to a colour in the middle of it, then the third time it gave me an error message and never fully installed. i'm pretty sure my graphics card is the RX 460 but i'm about to double check--i tried to but apparently i don't have it installed or something right now (another issue i just became aware of right now.) anyway, someone please help?? i can barely use my computer anymore.

Sounds like a gpu fault. Are you overclocked?

You would have set the overclock manually and thus you would know if you were.

You could try reflashing the bios on your video card. This is a somewhat complecated task but i have seen it fix gpus that crash and have drivers fail to install.http://www.tomshardware.com/faq/id-2288384/upgrade-gpu-bios.html
