Computer Keeps Shutting Off While Gaming

So recently my computer just randomly turns off and goes black screen 20-40 minutes after I start gaming then it boots back up. I've done some tests and it seems it only shuts down when I'm gaming. Today when it shut down it kept trying to boot back up, but failed 3 times before I had to turn the power supply off and on. My guess is my power supply is dying, but I've only had it for a year...Any ideas?
 
My specs are:

Gigabyte GA-Z87-HD3 ATX LGA1150
Vengeance® — 16GB Dual Channel DDR3 Memory
Seasonic S12II Bronze 620w
Intel Core i5-4670 Haswell Quad-Core 3.4GHz LGA 115
EVGA G-SYNC Support GeForce GTX 770 DUAL SuperClocked 4GB 256-bit GDDR5

 


My PC is not prebuilt. At idle my cpu is running at around 30° C. During gaming it runs around 42° C. I'm using CPUID HWMonitor to measure these temperatures. My 3.3v rail is at 4.980V, my 5v rail is at 4.980V, and my 12v rail is at 12.096V
