Computer Time Keeps Changing on its Own!!

I built a computer for a my father a few years ago and its had Windows 7 Ultimate on it up until a few weeks ago. When I had first installed Windows 7, the time would randomly change on its own and it drove me crazy trying to figure out what the issue was. The issue hasn't happened in a long time but I can't remember what I did to solve the issue, but now I've upgraded the computer to Windows 10 and the issue has returned. The time keeps changing randomly on its own several times a day. Windows 10 is installed on a new SSD that I added to the computer and the old Windows 7 HDD is in as a secondary drive. Anyone know how to solve this issue? It seems it isn't OS / software related as it only happens on this computer, even with fresh OS install, of a different OS, and on a new drive ...could it be the mobo / Bios time causing the issue?

Are the times to which your system time changes, seemingly random? Are the minutes synchronized with actual time but the hours different? If the minutes sync with actual time, then it's possible that something is modifying your time zone. Try disabling network time synchronization by typing "Date and Time" in Start and selecting the first option. Navigate to the "Internet Time" tab and select "Change settings..." Uncheck "Synchronize with an Internet time server."

If the time always seems to start with something like :00 or very close to :00, then you may be having a problem with your BIOS time. The CMOS battery may be failing, causing your time to reset after a while with the PC off. Replace your CMOS battery to remedy this.

Yes the time changes seem completely random.. yesterday it changed the date to 9/8/2015 and time was completely off with no relation to actual correction time...disabling internet time was the first thing I did, I manually set the time, it didnt work. Also, the computer isn't being shut down, stays powered on throughout the day and is put to sleep at night. and thought it was the CMOS battery first time around but it was a brand new mobo at the time and the battery was new, (mobo is 2 years old now) ..up until I recently added the SSD & Win 10 install, it hadn't changed the time on its own in over a year, so the system upgrades seem to have triggered it again so not sure its the battery but will change it out if nothing else works and see if fixes it

 
- maybe the bios is having problems reading the real time clock correctly. ,update the bios or reset it to defaults and reconfigure.

- remove overclocking software. Bad versions can tweak BIOS setting incorrectly

- hackers spoof internet time servers to confuse/change your computer time (various reasons)
You might change the time server from the default server.
