CPU overclocked in BIOS, but intermittently not overclocked in CPU-Z?

I recently noticed that my CPU is not always overclocked, even though it is set to be in the BIOS. About 4 times out of 5, the CPU will be overclocked, but the other ~20% of the time, it won't be. This is an Intel 3930K and ASUS Rampage IV Extreme motherboard.

I have the multipliers on all cores set to 44. Yet on relatively rare occasions, CPU-Z (and other Windows apps) report only the expected multiplier of 32. This is apparently not due to the CPU being at rest. If I run a CPU benchmark, one that ramps up the CPU to ~99% utilization, the results are consistent with a 32 multiplier in these cases. That is, they are about 37% lower than when the 44 multiplier is reported.

It is also apparently not because the processor is down throttling due to temperatures. My temps in the Intel Processor Diagnostic stay at least 37 degrees C below the max, even when overclocked.

I have both Turbo Boost and Intel Enhanced Speedstep Technology enabled in the BIOS.

I have my power plan in Windows set to High Performance.

How can I ensure that it is always overclocked?
