Can anyone tell me what creatures and assorted bag guys need +4 or greater
weapons to hit (in SoA and ToB)?

Demiliches, the Lesser Demon Lord, the final boss of ToB whose name is a
spoiler. Demogorgon might as well, I'm not sure. Anyone who casts
Improved Mantle does while the spell is in effect.

I'm pretty sure Demogorgon can be hit by +3 weapons. To clarify the
previous comment, Lesser Demon Lord takes +5s.

In Ascension I know the Fallen Solars and Melissan take +4 weapons to
hit, but I'm not sure if the unmodded game has the same weapon
requirements in effect (I seem to remember hearing people saying they
hit Melissan with +3s).

Anyway IMHO the only time it really matters is in Ascension finale.
There are two demiliches in the entire game and they're not that
dangerous with preparation. If I remember right, in unmodded game a
lich's Improved Mantle expires really soon after their Time Stop ends
(and you block all the symbols with Chaotic Commands, summons, good
saves, Remove Fear, whatever), and in modded game, it's always
Protection from Magical Weapons instead.
