CrystalDiskInfo shows "Caution" on my HDD. (C5 & C6) Need advice

Today I've been having lots of Bluescreens and slowed performance on my pc while play games or browsing. I decided to download CrystalDiskInfo which warned me about the [C5] (Current Pending Sector Count) and [C6] (Uncorrectable Sector Count) values of my HDD. Here's a screenshot of CrystalDiskInfo:

I suspect the drive is failing, what do you guys think I should do? Should I replace it ASAP? Or is there something I can do to repair it maybe with software or something? ( I apologize, I'm a noob to this stuff)
Anyways, any help is appreciated and I thank you for your time!

Well, I ran a fix with SeaTools bootable and it successfully repaired all the bad sectors. I'd really like to thank you guys once again for the advice, without it I would've probably written off this hard drive and would've had to buy a new one. With regards to the 50k hours, idk how it is so because I only built this pc in 2015 and its not like I leave it on 24/7 or use it all day, so that has to be an error.

As a last piece of advice, have a good back up plan for your data.

The conventional wisdom by tech experts on backups is known as the 3-2-1 method. Basically you want:

3 copies of any data you don't want to lose.
2 different mediums it's stored on (so 2 different drives in your computer, for example).
1 copy kept offsite, to prevent against disaster.
